Summary
Provides criteria, with examples, for auditing, evaluating, and benchmarking your office 5S program to provide an objective measurement for each element of 5S. The form is intended to be used for tracking the improvement of the area being 5S’ed over time. In addition, the form allows for the person responsible for auditing the area to document any ideas, suggestions, or comments.
Objective
Audit your 5S Program to encourage self reflection and ideas for continuous improvement.
Summary of Steps
- Go to the office area that you want to document
- Fill in the date of evaluation and the area concerned in their respective fields
- Begin auditing from left to right, reading each criteria and criteria detail
- Assign a score 1-5: 1 being poor, 5 being well done
- Give suggestions, ideas, or comments for improvement in the right hand field provided
- While filling out, keep in mind the definitions of the 5S’s:
- Sort
- Select what you need for the job and remove everything else
- Set in Order / Straighten
- Arrange remaining items for easy selection and use
- Shine / Sweep
- Root cause identification and removal of sources of disorder, clutter, and disorganization
- Standardize
- Maintain and continue improving the previous three S’s
- Sustain
- Elimination of effort to maintain 5S in the area
- Sort
- Add up and divide out your category subtotals to find individual scores
- For each category, add up each score to get your category subtotal
- The category subtotal is divided by the following numbers
- Sort score divided by 6
- Set in Order / Straighten score divided by 6
- Shine / Sweep divided by 5
- Standardize divided by 5
- Sustain divided by 4
- Total “category subtotals” and divide to find your average 5S score
- In the bottom right field marked total put in the number equaling your category subtotals divided by 26 to average your 5S score
